What is the Dr. Boz Ratio (Glucose-to-Ketone Index), and Why is it Important?

This article defines the Glucose-Ketone Index (GKI), explains how to calculate it using blood glucose and ketone readings, and highlights its importance in measuring your depth of ketosis.

Know Your Ratio

Welcome to the most important metric in your metabolic healing journey. When you commit to overcoming Insulin Toxicity and transitioning from a sugar-burner to a fat-burner, simply guessing based on what you eat is not enough. You need to look inside your bloodstream.

That is exactly what the Dr. Boz Ratio (our simplified Glucose-Ketone Index, or GKI) does. It measures the microscopic battle happening inside you every single day: the battle between Glucose and Ketones for dominance.

🚨 CRITICAL MEDICAL WARNING: Nutritional Ketosis vs. DKA This guide is for achieving nutritional ketosis. If you are a Type 1 Diabetic, or an advanced, uncontrolled Type 2 Diabetic, you must be aware of Diabetic Ketoacidosis (DKA)—a medical emergency. DKA occurs when your body has dangerously high blood glucose (e.g., over 250 mg/dL) AND dangerously high ketones (e.g., over 3.0 mmol/L) simultaneously. If your numbers look like this, seek emergency medical care immediately. Nutritional ketosis, by contrast, is characterized by low blood glucose and moderately elevated ketones.

The Math: Traditional GKI vs. The Dr. Boz Ratio

In standard cancer research, scientists use the Seyfried GKI, which requires dividing your US glucose reading by 18 to convert it to European metrics before dividing by ketones.

To make this actionable for you to track every single morning, we use the Dr. Boz Ratio. We skip the complicated conversions. You simply take your standard US blood glucose reading and divide it by your blood ketone reading.

How to Calculate Your Number

You need a blood glucose meter (mg/dL) and a blood ketone meter (mmol/L). Urine strips do not work for this, as they only measure waste ketones, not the fuel your brain is actively using.

The Formula: Blood Glucose ÷ Blood Ketones = Your Dr. Boz Ratio

Example:

  • Your Blood Glucose is: 90 mg/dL

  • Your Blood Ketones are: 1.0 mmol/L

  • Calculation: 90 ÷ 1.0 = 90

The Scoreboard: Mapping Your Ratio to the ketoCONTINUUM

Your ratio is a direct reflection of where you are on the ketoCONTINUUM. It tells you when you are fighting "Brain Bugs" (sugar cravings), when you are adapting, and when you have unlocked deep cellular healing.

  • Over 120 (Sugar Burner): Glucose is dominating. Your body is running on sugar, insulin is high, and your Brain Bugs are likely screaming for carbohydrates. You are not in ketosis yet.

  • 80 to 100 (Consistently Keto): You are officially flipping the switch! This is a great maintenance zone. Your body is learning to burn fat, and your insulin resistance is beginning to drop.

  • 40 to 80 (Advanced Healing): The magic zone. At this stage, you will experience sustained weight loss, dramatically reduced inflammation, and incredible mental clarity. You are healing, "One Ketone At A Time."

  • Under 40 (Autophagy): Peak cellular recycling. Ratios this low usually require strict fasting. Your body is aggressively cleaning out old, damaged cells, repairing your immune system, and maximizing mitochondrial efficiency.

Advanced Variables: Why Did My Number Spike?

If your diet is perfect but your Dr. Boz Ratio is stubbornly high, do not panic. Your body is not a machine; it is a dynamic biological system. Look out for these hidden variables:

  • The Dawn Phenomenon: Never test the second you open your eyes. Your body naturally surges cortisol in the morning to wake you up, which tells your liver to dump glucose into your blood. Wait 1 to 2 hours after waking for an accurate baseline.

  • The "Exogenous Ketone" Illusion: Taking MCT oil, ketone esters, or ketone salts will artificially flood your blood with ketones, making your ratio look incredible. However, these are borrowed ketones. You are burning the fat you drank, not the fat on your body. The ratio must be earned, not bought.

  • Hormonal Fluctuations: For women, the luteal phase of the menstrual cycle (the week before your period) involves a massive spike in progesterone. This temporarily induces insulin resistance, naturally raising glucose and stalling your ratio. Give yourself grace during this week.

  • Stress, Illness, and Exercise: High physical stress (like an intense heavy lifting session), emotional stress (poor sleep), or an active immune response (fighting a cold) will trigger a cortisol response, elevating your blood sugar.

The Reset Protocol: How to Fix a High Ratio

If you are stuck, you need an intervention. Dr. Boz has designed specific protocols to break metabolic resistance. Use this tiered decision tree to course-correct:

Level 1 Intervention: The 12-Hour Fast

  • When to use it: You ate a hidden carb, or your ratio is hovering between 100 and 120.

  • The Action: Stop eating. Consume nothing but water, black coffee, or plain tea with unflavored salt for 12 straight hours. Let your insulin drop naturally.

Level 2 Intervention: The "Fat Fast"

  • When to use it: You are consistently stuck above 80, struggling with intense cravings, and cannot seem to fast without feeling miserable.

  • The Action: For 24 to 48 hours, eat only pure fats. The classic Dr. Boz tool here is eating Brown Butter Bites (or bacon/macadamia nuts). This floods your body with fat to create ketones while keeping insulin completely flat because protein and carbs are virtually zero.

Level 3 Intervention: The Sardine Fast

  • When to use it: You have severe insulin toxicity, your weight loss has stalled for weeks, and your ratio refuses to drop into the healing zone.

  • The Action: The ultimate metabolic reset. For 72 hours, you eat nothing but sardines packed in water or olive oil, drinking plenty of water with salt. It is metabolically rigorous, deeply anti-inflammatory, and guaranteed to drop your ratio. (Refer to the ketoCONTINUUM for full Sardine Fast guidelines).
